The district's athletic program shall be free from discrimination and discriminatory practices prohibited by state and federal law. Any complaint alleging discrimination in the district's athletic program shall be filed in accordance with the appropriate complaint procedures. The form below is to document incidents of racial discrimination, harassment, and/or hazing that occur at high school sporting games or sporting events. Anyone who experiences, witnesses, or becomes aware of such behavior is encouraged to complete and submit this form to the school principal as soon as possible after the incident.
